Position Highlights:

RN Care Managers provide clinically-based care to support patients during and after their acute hospitalization and/or emergency department encounter.

  • Responsible for the overall accountability for care coordination and discharge planning for patients within the assigned caseload.
  • Collaborates with other members of the health care team to identify appropriate utilization of resources.
  • With the patient, family, and health care team, creates a discharge plan appropriate to the patient's needs and available resources including, but not limited to, rehab, long term care, home health, hospice, infusion and wound care.

CM Team-

RN Care Managers and Social Work Care Managers work together to ensure both clinical and social needs are addressed. The care managers are assisted by a group of resource associates who help pull together some of the final pieces of a discharge plan.

Assignment & Hours-

Care managers support all acute inpatient areas of the hospital as well as the ED. Each unit has assigned care management staff. Float care managers help cover absences and surge areas. RN Care Managers work 0800-1630 and may be assigned up to two weekends in a six-week period. Will be supporting Salem Hospital and West Valley locations

Training-

New hires go through a very detailed orientation. The training period is around eight weeks, allowing for thorough preparation for the role.

Supervision-

The care management leadership team is made up of three supervisors and a manager, all of whom have previous experience working as unit care managers at Salem Health. The RN Care Manager is supervised by an RN Supervisor.

Background/Expertise:

  • Requires ADN, BSN or MSN required.
  • Minimum of 1 year experience required. Care management experience preferred.
  • Current unencumbered licensure to practice nursing as a Registered Nurse in the State of Oregon required.
  • Basic Life Support (BLS) required.
  • Typing ability: Minimum 40 WPM.
  • Ability to work independently.
