Job Summary:

MedStar Health is looking for a Patient Navigatorto join our team!

Responsible for the coordination of patient care and access to services, provides materials and serves as a liaison with members of the patient's health care team as well as provides administrative support to the department physicians and staff. Functions as a patient advocate, guides patients and families throughout the scheduling, pre and post procedure process.

Primary Duties:

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.

  • Serves as navigator for patient and family members through coordinating and scheduling care with all members of the healthcare team including physicians, clinic support staff and other members of the healthcare team to achieve care goals for the patient.

  • Coordinates patient's appointments including office, specialists, testing, pre-operative, post-operative, procedural and/or surgical appointments.

  • Functions as a liaison between patients, families, health care team and community resources. Advocates for identifying and eliminating barriers to care.

  • Handles referral calls to ensure streamlined access to care. Ensures patient referral process for specialty services is coordinated, anticipating patient and family needs throughout the process.

  • Provides clerical and administrative support to physicians/department including answering the telephones, relaying messages, and transmitting information to the appropriate personnel. Composes and types the correspondence of various clinical and non-clinical documents. Manage physician's calendar and coordinates meetings as needed.

  • Schedules procedural and/or surgical cases and admissions for the department. Communicates posting information to physician, nurse and administrative staff within the department, as appropriate. Communicates scheduling information, including information regarding surgical and/or procedure schedule, MedStar Health facilities, policies and instructions to patients and families.

  • Obtains and uploads into the electronic medical records all previous medical records relevant to patient care including all surgical records, radiology and imaging reports and disks, pathology, cultures and lab work, etc.

  • Communicates referral requirements, patient's financial responsibility and other pre-visit information, as appropriate, including necessary procedures/examination preparations, e.g. NPO after midnight, according to departmental policies and procedures.

  • Appropriately and accurately scans and indexes documents, using correct naming conventions, into patient EHR.

  • Provides support services to patients and medical staff.

  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts.

  • Participates in multi-disciplinary quality and service improvement teams.

Qualifications:

  • Associate's degree required; bachelor's degree preferred.

  • CONSIDERATION WILL BE GIVEN TO AN APPROPRIATE COMBINATION OF EDUCATION/TRAINING AND EXPERIENCE required

  • 1-2 years of experience in a medical clinical office is and previous experience as a patient registration associate or Administrative Assistant preferred

This position has a hiring range of $22.04 - $38.41
