FTE: 1.0, Shift: Day, Schedule: 8-hour shifts, rotating weekends

Position Summary

The Care Coordinator Associate is part of an interdisciplinary care team consisting of physicians and clinical professionals to assist in the cross continuum approach to hospital transitions. This position will be a key component to achieving collaborative practices that exemplify MultiCare Health System's commitment to patient centered care, community engagement, and focus on transition of care for patients with physical and behavioral healthcare needs.

The ideal candidate will have excellent customer service skills and a collaborative approach to ensure smooth throughput and care transitions for patients. The Care Coordinator Associate must be adept in technology to utilize multiple data platforms and troubleshoot as needed. In addition, this role requires organizational skills and familiarity with scheduling, insurance and authorizations.

Requirements

  • Post-secondary coursework, preferably in a health related field
  • Bachelor's degree highly desirable
  • Minimum of 2 years experience in a health care setting, preferably as a Health Unit Coordinator (HUC) or in a clinical role such as medical assistant, certified nursing assistant or EMT
  • Experience in utilization management, community resources, and/or financial and quality issues affecting patient care desirable

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $20.47 - $29.45 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
