Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty, and addictions. We hire people who are skilled and passionate to meet our mission of ending homelessness through outcome-based strategies that support personal and community transformation. Central City Concern is looking to hire On-Call Primary Care Providers (NP, PA, MD, DO) Schedule: On-call shifts include Monday - Friday shifts in primary care and Saturday shifts in Quick Care. In both settings, the Blackburn Center offers Office Based Opioid Treatment (OBOT) services. As an on-call primary care provider, you will use your strong background in mental health, addiction medicine, and health care to offer integrated, client centered, trauma informed care to homeless and underserved populations. You will help support patients in achieving their housing, health, and resiliency goals. Using your background with the client population, you will collaborate with staff to determine health histories and continue to formulate this history with appropriate documentation and record keeping. Your ability to consider impacts of structural racism and oppression on individual client outcomes, inform your use of de-escalation strategies and your daily practice.
