Job Summary The Medical Social Worker IV performs comprehensive psychosocial assessments and formulates therapeutic plans for patients/families affected by chronic kidney disease who receive services from renal /dialysis clinics throughout Cook County Health (CCH). The Medical Social Worker IV assists in developing a care plan to meet patients' needs by participating in patient/family and staff meetings and in-services that are directed at supporting and maximizing the social and psychological functioning and adjustment of a renal/dialysis patient. The Medical Social Worker IV collaborates with a multidisciplinary team for provision of comprehensive social work services and documents those services according to facility policies that meet Medicare and Quality Assurance and Performance Improvement (QAPI) requirements. Typical Duties * Provides on-going comprehensive psychosocial assessment and counseling services to chronic kidney disease patients and their families to promote their quality of life * Provides psychotherapy, crisis intervention, psychosocial support and referral services * Assists with coordination of care including transportation services, durable medical equipment, advance directives, home health, housing, food and nutritional support, referrals to providers of health care coverage, and obtains documents to support care coordination and any other social service needs * Facilitates transfer process and placements to community dialysis centers and transplant centers * Works closely with the interdisciplinary treatment team in planning for patient care and regularly participates in team meetings * Initiates and/or participates in patient/family and/or staff orientation, education, such as developing educational materials and in-service programs for staff * Advocates patients' needs within CCH and with other local, state, and federal agencies and programs for access to services * Participates in departmental staff meetings * Maintains social work documentation and actively monitors patient data for programs. * Participates in all QAPI activities and reports to appropriate regulatory agencies * Keeps up with research, literature, developments in nephrology social work * Attends and participates appropriate continuing education institutes and conferences * Performs other duties as assigned Minimum Qualifications * Master's degree in Social Work (MSW) from an approved school of social work is required (Official copy of transcript is due at the time of interview) * Active license in social work in the State of Illinois (LSW) is required * Two (2) years working as a social worker in a clinical or medical setting is required * One (1) year of dialysis unit or transplantation experience is required Preferred Qualifications * Experience performing crisis intervention is preferred * Experience and Basic knowledge of Cerner applications is preferred * Certification as a nephrology social worker is preferred * Bilingual is preferred VETERAN PREFERENCE PLEASE READ When applying for employment with Cook County Health, preference is given to honorably discharged Veterans who have served in the Armed Forces of the United States for not less than 6 months of continuous service To take advantage of this preference a Veteran must: * Meet the minimum qualifications for the position. * Identify self as a Veteran on the employment application by answering yes to the question by answering yes to the question, "Are you a Military Veteran?" * Attach a copy of their DD 214, DD 215 or NGB 22 (Notice of Separation at time of application filing.
