Job Overview:

The Nutrition Support Dietitian will provide expert nutritional guidance and support to patients receiving home infusion therapy. Clinical monitoring of a patient s nutritional status including labs, weights, and any diet advancement will be considered to optimize nutrition related outcomes and meet patient and provider goals of care. This position requires strong communication and organizational skills, as well as the ability to work closely with a multidisciplinary team. After successful completion of training, will be able to work from home.

Qualifications:

Required:

  • Bachelor s degree in Nutrition or Dietetics.
  • Minimum of 5 years of experience as a clinical dietitian, with a focus on nutrition support.
  • In-depth knowledge of enteral and parenteral nutrition support therapy.
  • Strong understanding of medical nutrition therapy and its application in conjunction with disease-specific treatment plans.
  • Excellent communication and interpersonal skills, with the ability to effectively educate patients and collaborate with healthcare professionals.
  • The ability to work in a fast-paced environment and handle multiple priorities.
  • Proficient in using nutrition software applications, electronic medical records, and other relevant technology.
  • Demonstrated commitment to continuous learning and professional development in the field of nutrition support.

Preferred:

  • Certified Nutrition Support Clinician certification preferred.
  • Experience in a hospital or other healthcare setting is highly desirable.

Certifications and Licenses Required:

  • Registered Dietitian through the Commission on Dietetic Registration.

  • After successful completion of training, will be able to work from home.

Essential Functions:

  • Oversee all aspects of the nutrition support program, including patient monitoring and program compliance.
  • Collaborate with sales team to optimize referrals with key stakeholders.
  • Conduct comprehensive nutrition evaluation and communicate appropriateness for home PN therapy upon admission into service.
  • Review clinical documentation to support billing/reimbursement.
  • Provide comprehensive nutritional assessments and develop individualized nutrition plans for patients receiving home infusion therapy.
  • Collaborate with physicians, pharmacists, and other healthcare professionals to optimize patient care and outcomes.
  • Monitor and evaluate patients' nutritional status and make necessary adjustments to their nutrition plan as needed.
  • Conduct regular follow-up visits with patients to assess progress and address any concerns or questions they may have.
  • Educate patients and their families on appropriate nutrition and dietary modifications to support their medical conditions.
  • Maintain accurate and thorough documentation of patient assessments and interventions.
  • Stay up-to-date with the latest research and advancements in the field of nutrition support and home infusion therapy.
  • Assures patient, hospital and referral source satisfaction. No complaints registered.
  • Responsible to send lab results, clinical updates, recommendations and any concerns to provider along with current TPN formula for renewal.
  • Understands and adheres to the company policy and requirements. EG: Snow day and sick time.
