Job Summary:
Full-Time Post Kidney Transplant Coordinator:. The ideal RN candidate has great communication skills, is flexible, and has a strong patient-centered approach to care. Transplant experience highly preferred.
Monday - Friday, with occasion on call requirements. Hybrid position - candidate must be flexible and will work 2-3 days per week depending on clinic schedule. On call one week approximately every 14 weeks.
EXPERIENCE AND EDUCATION
Required
- Education
Associate's Degree in nursing required. and
- Experience
3-4 years of Medical/Surgical or Intensive Care experience.
- Licenses and Certifications
(NLC) NURSING LICENSURE COMPACT-BNE Current Texas RN (or BNE Compact State) license required. Upon Hire and
(BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC). Upon Hire and

JOB DUTIES
- Performs and documents physical, psychosocial, and emotional assessments. Takes all assessments into consideration age specific criteria.
- Anticipates patient's and/or patient's family's expectations and needs and utilizes them in conjunction with assessment to develop appropriate plan of care for patient. Maintains and utilizes competencies per unit expectations. Utilizes critical thinking skills in planning and prioritizing nursing care.
- Identifies and implements appropriate nursing interventions based on assessment. Completes documentation of interventions. Coordinates interdisciplinary plan of care. Confers with members of health care team, sharing information and observations of patient and/or patient's family, to ensure optimal patient care.
- Prioritizes nursing interventions based upon assessment. Identifies changes in condition as evidenced by assessment, labs, x-rays, etc. Reports changes and takes follow-up action as appropriate.
- Evaluates implemented interventions and modifies care according to evaluation. Documents interventions and modifications to plan of care.
- Adheres to appropriate policies, procedures, clinical pathways, and nationally accepted standards of nursing practice while delivering care to patients. Suggests practices to enhance nursing care based on current literature and research.
- Provides answers and/or education to patients and/or patients' families. Completes documentation of all education provided. Obtains feedback and evaluation of understanding from patient and family. Ensures comfort level of post-discharge care. Demonstrates effective communication with children, adolescents, adults, and older adults.
- Welcomes and orients patient and family to unit. Encourages patient and family participation in care. Reviews plan of care with patient and family. Explains special diets, tests, procedures, and medications. Makes rounds on patients.
- Reports errors promptly and takes appropriate follow-up action to rectify errors. Provides supervision of, and direction to, unlicensed support personnel and nurses new to unit. Delegates duties appropriately and within legal parameters.
- Responds to patient's and/or family's complaints in appropriate and effective manner. Reports complaints to Clinical Nurse Manager or designee in timely manner. Utilizes chain of command to resolve issues. Serves as advocate to patient and family.
- Participates in performance improvement activities as directed. Attends at least 80% of scheduled staff meetings and 100% of mandatory educational sessions.
- Performs other duties as assigned.
