Purpose of Job This casual position is responsible for the delivery of individualized assessment, care planning, intervention and discharge of Speech Therapy. Job Functions
- Performs discipline specific evaluations, OASIS assessments, care planning, interventions, documentation of care, and discharge of client cases according to RMC, agency, and professional standards.
- Provides current and quality treatment, utilizing Ridgeview, agency and professional resources to promote continued quality care.
- Communicates with the client, health care providers, insurer and others to ensure that client needs are addressed and authorization is obtained.
- Serves as a client advocate to obtain referrals, to advise and utilize appropriate Ridgeview, agency and community resources to meet client needs.
- Exhibits working knowledge of hours of operations, customer base, department operations, visit procedures, and chart audit procedures.
- Demonstrates competencies in the specialized are of Home Health, and utilization of reimbursement and regulatory guidelines for the provision of safe and effective care.
- Provides effective supervision of home health aides and other applicable caregivers when Speech Therapy is provided.
- Participates in meetings, care conferences, and quality improvement initiatives as directed.
- Provides professional and courteous service to internal and external customers.
- Performs other duties as assigned.

- Ability to meet and maintain the necessary background checks as aligned with position functions
- Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above

License/Certifications
- Licensed Speech Therapist, valid in Minnesota.
- Current Healthcare Provider CPR Certification
- Valid MN Driver's license and meets Ridgeview's age and insurability requirements.
