Position in this function is responsible for providing expert clinical and administrative leadership to teammates within a given department or multiple departments to ensure efficiency of operations and functions. Under the supervision of a clinician, this teammate is responsible for ensuring that all policies and procedures are followed related to patient clinical care and safety. Responsible for performing operational duties as required under the supervision of the Lead clinician, site administrator or designee.

Primary Responsibilities:



  • Team Oversight


    • Directly supervises RNs, LVNs, and Health Advocates, focusing on daily operational responsibilities
    • Works closely with the director to support the Optum CA ACO Commercial and Medicare programs


  • Project and Task Management


    • Ensures successful execution of projects by monitoring progress consistently
    • Identifies and mitigates risks or challenges early in the process
    • Ensures timely completion of tasks across the team


  • Collaborative Environment and Team Support


    • Fosters a collaborative environment, providing guidance and support to team members
    • Acts as a working supervisor, participating in daily tasks alongside the team


  • Focus on Quality and Compliance


    • Proactively identifies and addresses gaps to meet health plan and CMS KPIs
    • Conducts regular documentation audits to ensure accuracy, quality, and compliance


  • Resource and Workflow Management


    • Manages resources, schedules, and workflow in collaboration with the director to promote efficiency
    • Optimizes team workflows to support high-quality patient care


  • Patient Education and Oversight


    • Leads patient education initiatives and ensures that patients are supported in understanding their care
    • Provides oversight on the team's patient interactions, ensuring quality and satisfaction


  • Team Meetings and Communication


    • Regularly conducts team meetings to align on goals, share updates, and address any issues
    • Communicates effectively with the team, providing clear direction and fostering a supportive environment


  • Functions as a resource and serves as a liaison between administration, clinicians, teammates, other departments, patients and vendors
  • Participates in Nursing Leadership, Nurse Peer review and other committees as needed
  • Participates in educational programs to enhance nursing and management knowledge
  • Trains teammates in collaboration with the director

Required Qualifications:



  • Graduate from an accredited school of Nursing
  • Current California RN Lic
  • Care management experience
  • ACO Commercial and Medicare program experience
  • Intermediate level of experience with Microsoft Suite



Preferred Qualifications:



  • Prior supervisory experience
  • Prior ambulatory care experience

Residents Only: The salary range for this role is $70,200 to $137,800 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
