Sentara Medical Group has a full-time opening for an APP (NP or PA) to join our Sentara PACE (Program for All-inclusive Care for the Elderly) team located in Norfolk, VA.

PACE is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals who would prefer to remain at home with services coordinated by Sentara PACE. The program provides supportive care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation.

Position Highlights:

  • Average number of patients a day is 5-10

  • 1:3 call schedule, weekdays 5pm-8am, weekends Friday 5pm-Monday 8am

Minimum Requirements:

  • Active NP or PA

  • Experience managing chronic conditions and working with a geriatric population preferred

  • Medicare/Medicaid experience preferred
