How You'll Help Transform Healthcare:
In collaboration with the health care team, the Discharge Planner serves as a patient advocate while developing a safe and appropriate discharge plan which provides the best possible disposition for the patient while utilizing resources effectively and efficiently.
- Provides assessments of patients and families to identify emotional, social and environmental strengths and problems related to their diagnosis, illness, treatment, and or life situation.
- Formulates, develop and implement a comprehensive discharge plan utilizing appropriate social work interventions. Coordinate the plan with the interdisciplinary team and make recommendations for further treatment for the patient to achieve his/her optimal level of functioning. Evaluate the process at appropriate intervals and modify the plan as necessary. Monitor length of stay.
- Arranges, procures, and co-ordinates patient/family post hospital needs, participates as a member of the interdisciplinary team. Plan for care at another level (SNF, ICF, assisted living, physical rehab, etc.) or reintegration into the home/community.
- Plan of care promotes compliance with all applicable regulations as well as consideration of appropriate resource utilization. Advocates on behalf of the patient. Utilizes electronic programs to facilitate placements and documents in these programs to meet documentation standards.
- Collaborates with community agencies and resources to develop a comprehensive knowledge base of available resources for patients and families. Networks with community resources to build strong working relationships with these agencies/resources. Maintains an extensive knowledge of benefits and regulatory guidelines to maximize utilization of resources and cost effectiveness.
- Meets documentation standards for quality and timeliness. Maintains confidentiality of patients/families medical information in accordance with department and hospital policies. Adheres to established productivity measures.
- Remain current with social work and health care developments. Attends educational offering provided by Carillion and in the community. Identifies social problems and advocates for change.
- Provides education to patients and families for issues related to adaptation of the patient's illness or situation.
Hours are: M-F 3pm-11pm
What We Require:
Education: Bachelors Degree in Human Services requiredor background in related field.
Experience: Experience in a health care setting preferred.Experience working in a mental health setting preferred.
Other: Must successfully complete orientation and competency validation for position. Demonstrate ability to think independently and problem solve, prioritize and multi-task. Ability and willingness to work as an integral member of a multi-disciplinary team.
