Job Summary: The Up Front Assessment Assessor and In Home Outreach Counselor (IHOC) is responsible for conducting urgent assessments of families referred to our Family Preservation Program for an Up Front Assessment (UFA). The UFA is voluntary and provides county social workers with valuable information on the parent/caretaker capacity, when considering issues related to mental health, substance abuse, and domestic violence. The goal is to prevent unnecessary out of home placement through more thorough assessment where detention is necessary, providing information to allow for more appropriate case plans to be developed. The UFA can help the county social workers determine what kind of services they may be beneficial to offer families. In addition to completing assigned UFA's, the assessor will carry a small caseload (2-6 clients) of Family Preservation clients in the role of an In-Home Outreach Counselor (IHOC). The IHOC is responsible for supporting Family Preservation clients. The IHOC offers in home counseling to families at least once a week. They are also responsible for providing in-depth case management services to ensure that families are adhering to the agreed upon service plan. Availability during evenings and weekends is essential for conducting UFA assessments and visiting Family Preservation Clients. This intermittent employee follows all Penny Lane Centers policies and procedures regarding the Heart Centered Culture, cultural sensitivity, and cultural awareness. The employee supports the agency mission and vision in their interaction with children, youth, families, and partner agencies. Requirements:
