We are seeking an empathetic and resourceful Care Coordinator to provide one-call resolution and personalized support to individuals and families seeking care. In this role, you will serve as the first point of contact, conducting screenings to assess needs and ensure patients are directed to the appropriate programs and levels of care. Your role is crucial in navigating waitlists, addressing barriers such as insurance or scheduling conflicts, and keeping patients engaged throughout the process.
Schedule: 1:00pm -9:30pm
Program Coordination
- Conduct initial 20-30 minute screenings to evaluate patient needs and determine appropriate care programs.
- Provide detailed information about available programs, levels of care, and what clients can expect during the process.
- Identify and address potential barriers such as insurance concerns, school schedules, or hesitations, ensuring clients feel supported and informed.
- Maintain regular communication with clients on waitlists, keeping them engaged and motivated while they await placement.
- Collaborate with the review team to transition clients smoothly and ensure their care plans align with their needs.
- Educate clients and families about insurance coverage, requirements, and resources to alleviate any financial concerns.
- Foster a compassionate and reassuring environment to build trust and long-term engagement with clients.
- Document patient interactions and follow-up actions accurately in the system.
Develop working relationships with interdisciplinary team members to ensure open line of communication regarding a patient's care. This includes but is not limited to:
- Inpatient, Outpatient, Residential Admissions and Patient Access departments
- Providers (Psychiatrists, Psychologists, APNPs, etc.)
- Outpatient operations
- Business Development
- Utilization Review
- Patient Financial Services
- Health Information Management

Education/Training Requirements:
- Bachelor's degree preferred. High School diploma required. Two (2) years of customer service and relevant experience required, preferably in a healthcare setting.
- Experience with computers and demonstrated proficiency with software platforms and EMRs.
- Knowledge of medical/psychiatric/chemical dependency diagnosis, treatment process, managed care, Joint Commission, preferred.
- Oconomowoc Only: Valid driver license preferred and must be granted insurable status by the Rogers insurance policy.
Compensation for this role is between $20.35 and $29.27, and placement in the range will vary based on years' experience, and education.
