The Wraparound Hospital-based Violence Intervention Program is under the UC Davis Medical Center, Patient Care Services Department. The role of this position is to support Wraparound patients' post-injury recovery and facilitate transition from trauma center services to community care by providing patient-centered case management and care transition interventions that address patients' post-injury concerns, enhance patient self-efficacy, and reduce post-injury inpatient and emergency department health service use.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position
- Bachelor's degree in related area or equivalent experience/training.
- Experience working with youth and young adults affected by violence or involved with the Justice System.
- Completion of Violence Prevention Professional Certification within 1 year of hire.
- Completion of CITI Basic course for Social and Behavioral Researchers and Staff within 3 months of hire.
- Completion of Mental Health First Aid training within 6 months of hire.
- Strong interpersonal and customer service skills.
- Ability to communicate and resolve issues effectively with a diverse population of patients, staff and physicians.
- Excellent analytical and problem-solving skills.
- Ability to develop solutions and recommend changes and follow through with implementation.
- Excellent written and verbal communication skills.
- Proficiency with Windows-based software including Microsoft word, Excel, Outlook.
- Knowledge of computer systems and software used in functional area.
Preferred Qualifications
- Experience connecting individuals to needed social services in Sacramento County.
- Experience providing mentorship for youth affected by violence.
- Applicants with personal experience in overcoming violence and violence-related injuries are encouraged to apply.
- Knowledge of intervention techniques and trauma-informed practices.
- Ability to speak and write in Spanish is strongly preferred.

The Trauma Prevention Program, within the UC Davis Medical Center Patient Care Services department, is responsible for fulfilling injury prevention requirements for American College of Surgeons Committee on Trauma (ACS COT) verification for Level I Pediatric and Level I Adult trauma centers. The mission of the Trauma Prevention Program is to prevent and reduce the leading causes of injury within the UCDH trauma center catchment area through education and outreach.

Work Environment
