We are seeking a talented and compassionate individual to join our CCS Care Coordination team! PSG/CIP is a multi-service community-based social services organization offering mental health and programming throughout eastern Wisconsin.

As our new Care Coordinator, you will be responsible for providing care coordination services to individuals with significant mental health and substance abuse diagnoses. Care coordination services incorporate the wraparound model of service delivery which supports in the home, community, and other areas of the individual's life and encourages a team approach to assist in meeting their needs. Care coordination will include a strength-based recovery and person-centered approach to service delivery.

KEY RESPONSIBILITIES OF THE CARE COORDINATOR:



  • Complete thorough strengths and needs assessments with clients during intake and continuing through services.
  • Analyze client situations, capabilities, and problems to determine services required to meet client treatment needs.
  • Develop, implement, evaluate, and revise goal-oriented and strength-based client Recovery Plan of Care and update as changes occur.
  • Facilitate team meetings with service providers and supports, and monitor progress of goals and services.
  • Develop client crisis/safety plans.
  • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
  • Develop a support team comprised of formal and informal supports.
  • Attend client/family court proceedings, psychiatry appointments, or other appointments which may require support or advocacy.
  • Safely transport clients as needed.


OFFICE LOCATION & HOURS: West Allis office. Some local travel is required to meet with clients at the home or community settings, with mileage reimbursement. Some flexibility for hybrid/remote scheduling. Hours will vary, but typically Monday-Friday between 9:00am to 5:00pm.

SALARY:

Candidates with a Bachelor's Degree: $23.00-26.00 per hour, based on experience

Candidates with a Master's Degree: $26.00 per hour

REQUIREMENTS:


  • Must possess at minimum a Bachelor's degree in Social Work, Psychology, Criminal Justice, or related.
  • Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
  • One year of experience working with clients with significant mental health or substance abuse diagnoses is preferred.
