JOB SUMMARY: The Pediatric Health Navigator (PHN) is responsible for bridging the gap between parents and providers through outreach and screening of pediatric patients for developmental delays and metabolic disorders using reliable and valid screen tools. Working closely with the Pediatric RN, providers and clinical leaders, the PHN ensures the availability of early childhood developmental screenings and works to support closing preventative care gaps for pediatric patients. Daily responsibilities include conducting outreach via phone calls and portal messaging to schedule appointments, respond to client inquiries, and address barriers to the completion of early developmental screening tools required for productive provider visits. MAJOR DUTIES AND RESPONSIBILITIES: Patient Focus * Provides outreach, education, coaching, and support to new and existing patients due for preventative early childhood development care and metabolic screenings. Communication * Utilizes best practices in motivational interviewing to support parental engagement. * Communicate clearly with parents, patients, care teams, and supervisors to convey pertinent information. * Responds to inquiries in a timely, courteous, and efficient manner. Workplace Computers and Equipment * Schedules medical appointments accurately according to organizational guidelines; inputs registration information when appropriate. * Collects, completes and indexes clinical information in advance of scheduled provider visits. * Accurately documents and routes care coordination and phone notes in EMR. * Utilizes required platforms (Microsoft Office Suite, EMR, CRISP, EZAccess, etc) appropriately and in accordance with HIPAA standards. * Performs all other duties as requested. Teamwork * Collaborate with other members of the care team for feedback and support. * Consistently positions self as resource for parents and the care team for successful screening for developmental delays SKILLS AND ABILITIES: * Excellent customer service skills * Good interpersonal skills * Excellent communication skills * Proficient in Microsoft Office Outlook, Word, and Excel * Attention to detail * Ability to learn quickly * Ability to adapt to change * Open to feedback * Team player * Able to follow through with delegated tasks * Bi-lingual skills a plus EDUCATION: * Required: High School Diploma or GED * Preferred: Some college EXPERIENCE: Required: One year experience working in a healthcare setting, preferably with pediatric patient populations. |
