Job Description

The Manager -Heart Failure Program is an advanced practice registered nurse (APRN) collaborates with physicians in managing acute and long-term medical needs of patient within the inpatient and outpatient setting, including assessing diagnosing, prescribing, treating, and educating heart failure patients under direction of the cardiologist. This position collaborates with physicians, administration, and others to develop and maintain processes for heart failure clinic based on data and evidence-based principles and development of growth strategies including the obtaining and maintaining of certifications/accreditations. This role works closely with physicians and other clinicians in a team approach. The APRN orders and interprets appropriate tests i.e., labs, EKG, under Physician supervision. This role also leads the heart failure team and works toward strategic and programmatic growth for the heart failure clinic.

Responsibilities

Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination. Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks. Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Leads heart failure team including medical assistants, nurses and APPs. Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care. Collaborates with physicians in managing acute and long-term medical needs of patient within the inpatient and outpatient setting, including assessing diagnosing, prescribing, treating, and educating heart failure patients under direction of the cardiologist. Provides monitoring and continuity of care between visits according to heart failure treatment plan including triaging patient calls/e-mails. Works in collaboration with physicians, administration, and others to develop /maintain processes for heart failure clinic based on data and evidence-based principles and development of growth strategies including the obtaining and maintaining of certifications/accreditations. Works in collaboration with committees to reduce heart failure readmission rates through data review, education, and regular meeting attendance. Works in collaboration with all departments (dietary, pharmacy, EP, Mayo, etc.) to provide quality care for heart failure patients. Functions as a role model for current and new staff in all areas of the hospital. Maintains current knowledge/certification/licensure. Pursues professional growth and development. Is responsible for department's operational excellence; ensures department delivers quality services in accordance with applicable policies, procedures, and professional standards. Effectively communicates departmental, organization, and industry information to staff.Assist in other duties as assigned to support the operational needs of the department and organization. May be required to remain on campus immediately before, during, and after severe weather and/or disasters.
