General Purpose of Job:As part of the Population Health Service Organization (PHSO) team, the Resource Coordinator will play an integral role in Sparrow Health System's (SHS) ambulatory care coordination efforts, specifically addressing social needs that exist beyond the scope of traditional healthcare services. This position will work closely with Sparrow Medical Group (SMG) practices to assist patients in accessing the essential information and services needed to attain an optimal quality of life. The Resource Coordinator will be responsible for connecting Sparrow patients with community resources where needed, aiming to close gaps and remove barriers related to the Social Determinants of Health (SDOH). He/she will serve as the outreach coordinator, educator, and conduit to internal/external services. Common Duties and Responsibilities:Working as the member of a centralized team, the Resource Coordinator will work closely with SMG Primary Care Physicians (PCPs) and PHSO RN Care Facilitators (RNCFs) to effectively manage Sparrow patients across the care continuum. The Resource Coordinator will act as a support mechanism for the larger operations and appropriately work with/triage patients to the next needed level of care. - Improves access to health care services through in-person interactions, telephone contacts, and coordination of referrals
- Provides an interdisciplinary needs assessment and identifies services needed to meet the objectives of the patients and caregivers
- Seeks, establishes and maintains favorable relationships with hospitals, social service agencies, physicians and other health care providers, community/professional groups, and other possible referral sources as well as professionals in the community at-large
- Collects, enters, manages and reports on the data related to referral patterns
- Acquires and maintains educational resources such as brochures, handouts, or other materials
- Assists in curriculum development to educate both internal and external stakeholders on the services available through the team
- Interviews and assists patients to understand current needs and resources available, connecting with the patient's Primary Care Physician (PCP) when a care plan is created or changed and follows up to make sure the plan is adhered to
- Maintains frequent contact with community agencies and participates in health fairs, Mobile Health Clinic outings, and other meetings when needed
Job Requirements
General Requirements |
* Valid driver's license * Current Community Health Worker (CHW) through Michigan Community Health Worker Alliance (MiCHWA) or Henry Ford's curriculum OR must be completed within one year of hire. |
Work Experience |
* Proficiency with MS Word, Excel, Project, and Power Point required. Other software includes Visio, Enterprise Project Server, SharePoint. * Experience working in ambulatory care management programming - preferred |
Education |
* Bachelor's degree in Human Services, Social Work, healthcare, or other related field; OR Associates degree in Human Services, Social Work, healthcare, or other related field and a minimum of two years of experience in case management, discharge planning, or relevant clinical experience; OR High School Diploma/GED and minimum four years of experience in case management, discharge planning, or relevant clinical experience * MA, LPN, CHW, BSW or similar clinical role - preferred. * Master's degree in a health-related field - preferred |
