Licensed Clinical Social Worker, LCSW, LMFT, LPC, and LMSW for Intensive Outpatient Program, New London, CT. Responsibilities Intensive Outpatient Program Provides clinical care and management of a caseload of patients, many of which are in a sub-acute phase of treatment preceded by an inpatient stay or emergency room visit. Referrals are also obtained from community providers for those needing a higher level of care and self-referred individuals. Practice within collaborative interdisciplinary team of providers (MD, APRN) and support staff to provide a group therapy utilizing evidence-based modalities. Individual, family, and crisis interventions are provided by the clinician as needed. Provide discharge planning for continuity of care and case management connecting patients to community resources.
Qualifications EDUCATION MSW, MA, MS, MFT EXPERIENCE Two (2) years experience preferred, will consider well qualified candidates if less than 2 years practice experience LICENSURE Licensed as one of the following: LCSW, LMFT, LPC, or LMSW SPECIAL SKILLS Knowledge of clinical measures such as PHQ-9, GAD-7, CSSRS, etc. and ability to administer to patients to assess progress and patient outcomes. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment. Knowledge of health promotion, health risk identification, and patient education principles and techniques. Develop and use skills in the implementation and use of the electronic medical record (EMR) applications. Ability to analyze complex data and draw conclusions. Ability to interpret, adapt, and apply guidelines and procedures. Ability to establish/maintain effective working relationships with patients, clinical staff, and community agencies. Population expected to work with currently include. Adults 19- 64 and Geriatrics 65 + years of age. In the future program could expand to Pediatric and Adolescent populations.
