Duties/Responsibilities: Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers Navigatemanagedcareplansforcommunityservicesandprograms Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems Proactivelyidentify methods toimprove ArchWellHealth's approachbased onfeedbackandregularlyconductedsurveys Support advance care transitions with members and their families Assist members with access to state-based prescription programs and other benefits RequiredSkills/Abilities: Experiencesupportingpatientswithneedforsocialservices Experiencewithscreening,assessment,andplanningforcommonsocialservicesneeds Workingknowledgeofdifferentialdiagnosisofcommonmentalhealthconditions Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities Highlyempathetic,non-judgmental,andopen-minded Experienceinacollaborativeteamenvironment EducationandExperience: FullylicensedLMSWinthedesiredStateofpractice Master'sdegreeinSocialWork 1+years'experienceinclinicalsocialwork Aproblem-solvingorientationandaflexibleandpositiveattitude Missiondrivenandmotivatedtojoinanorganizationthatwilltransformthewaywedeliveraccessible,clinicallyexcellentcaretoseniors ProficientPCskills FluencyinSpanishorotherlanguagesspokenbypeopleinthecommunitiesweserve(where necessary) ArchWell Health is an equal opportunity employer.
