Job Description: Under general direction, the Patient Connection Hub (PCH) Access Coordinator will actively monitor and address all referral and appointment requests using general clerical and customer service skills including, but not limited to, organizing requests from multiple locations and communicating status of appointments/referrals. The PCH Access Coordinator serves as the point of contact for patients, referring physicians and their office staff, and the various Baptist Health System departments for all access related requests. The goal is to deliver a high-quality and integrated Baptist experience; ultimately ensuring that patients receive the correct care, from the appropriate provider in right timeframe.
