Description
The Nurse Care Coordinator (Certified Community Behavioral Health Clinic) leads care coordination services, conducts physical health and preventative care screenings, monitoring and follow up for project populations. Overall ensures the delivery of services that meet the needs of the client through the coordination of care.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Monitors and ensures that health care information collected at intake and other intervals is distributed to staff, psychiatrist and other medical personnel as necessary.
- Gives treatments to patient following physician's instructions. Conducts preventive care screening and follow up, including BMI, weight, diabetes and may conduct tobacco and alcohol screening, suicide and depression screening and medication adherence screening and management.
- Utilizes service data to monitor and coordinate CCBHC consumers' healthcare integration with collaterals.
- Collaborates as needed with pharmacies, mental health, substance use, medical providers, and collateral supports, such as counselors, outreach teams, courts and primary care providers.
- Works closely with all consulting psychiatrists and medical doctors as assigned. This may include but is not limited to: medication administration, scheduling appointments and coordinating care as indicated by the consumer's service plans.
- Attends to any medically related emergency or crises providing intervention as appropriate while taking charge of the situation making such determinations about other interventions as needed.
- Provides staff and consumer training as requested on such topics as medication compliance, human sexuality, first aid, CPR, naloxone administration, the Heimlich Maneuver and others.
- PMP monitoring for controlled substances prescribed
- Monitors vitals for those referred to MAT induction
- Provides on-site groups for Nicotine Cessation/Nicotine Replacement Therapy (NRT), Reduction of Risk Behaviors related to STI/HIV/Hepatitis and opioid overdose prevention
- Meets required billing targets and completes all documentation within assigned time frames.
- Other duties as assigned.
- Schedule: Monday - Friday 8:30am-5:00pm
MARGINAL DUTIES & RESPONSIBILITIES
- Occasionally visits homes to determine patient and family needs, develops plan to meet needs, and provides nursing services.
- May be required to work flexible schedules and be available to work on weekends or evenings as required.
EDUCATION
- Associates or Bachelor's degree from an accredited school of Nursing required.
- Master's degree from an accredited school of Nursing preferred.
EXPERIENCE
- Two (2) years experience working with persons diagnosed with mental illness preferred.
- Ability to synthesize and summarize information and make judgments regarding patient care.
- Must communicate effectively both verbally and in writing in order to provide information to patients and healthcare personnel.
- Demonstrated ability to function independently.
- Computer skills required; knowledge of database functions and maintenance necessary.
SKILLS/CERTIFICATIONS
- Licensed Registered Nurse required.
- Current and valid driver's license preferred.
- Daily access to a personal vehicle is preferred. Must obtain and maintain liability insurance coverage. Subject to validation every 6 months.
- May be required to provide transportation for members of the program using a Thresholds provided vehicle or personal vehicle.
