opportunity with Yoh's client located in Palo Alto, CA

Top Skills Should You Possess:

• Inpatient Clinical Pharmacy Experience required
• Schedule is completed in two (2) month increments - must be comfortable with the below:
  
  
• Day: 9:00am-5:30pm or 8:30a-5pm (depends on service line)
• Evening: Every 2 weeks, 1:00pm-9:30pm
• Weekend rotations- 7 day operation so rotation is required; 4-6 days of weekends every two months (8:00am-4:30pm)

What You'll Be Doing:

• Inpatient service lines: cardiology, heart failure, neurology, medicine, geriatrics, psychiatry, hematology, oncology, others
• Post-discharge transitions of care clinic
• Admission and discharge medication reconciliation
• Patient/caregiver education
• Ensure medication access for patients
• Precepting of residents/students
• Prior Authorization Consultation Service
• Perform projects and assignments as assigned

What You Need to Bring to the Table:

• Doctor of Pharmacy degree and Clinical Pharmacy residency preferred or equivalent practice experience
• California Licensed Pharmacist required
• BLS Certification
• Three to five (3-5) years of experience in a Inpatient Clinical Pharmacist role required
• Excellent verbal and written communication skills to convey medical and research-related information to research subjects of all ages and backgrounds
• Excellent computer skills
