What You Need to Know

RN Plan of Care Coordinator Responsibilities:

  • Responsible for the quality of home health patient Care Plans in collaboration with assessing clinician and coding coordinator.
  • Utilize referral documents, orders, comprehensive assessments, and OASIS documentation to edit/build a detailed and individualized POC that demonstrates home health eligibility and skilled need.
  • Maintain up to date status information on any assigned cases that are unable to be completed due to outstanding information.
  • Notify leadership of problematic trends identified during case reviews.
  • Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications.
  • Assist with other chart audits/tasks as assigned.
  • Assist clinical/branch staff with interpretation of regulations and guidelines.
  • Participate in departmental staff and in-service meetings, as required.
  • Comply with applicable legal requirements, standards, policies, and procedures including but not limited to those within the Compliance Process, Code of Conduct, and HIPAA requirements.
  • Participate in required orientation, Compliance and HIPAA training programs. Report concerns and suspected incidences of non-compliance to the supervisor, administrator, compliance officer and/or the compliance hotline.

Qualifications

RN Plan of Care Coordinator Qualifications:

  • An associate degree in nursing with current unencumbered licensure to practice nursing in the state of residence required. Bachelor's degree preferred.
  • Minimum 1 year experience in the health care required. Home health experience preferred.
  • Computer skills required.
  • Knowledge of Microsoft Teams and Outlook preferred.
  • Knowledge of Homecare Homebase EMR system preferred.
