The Department of Cardiology at Boston Children's Hospital Benderson Family Heart Center is known around the world for its superior patient care and clinical outcomes. Drawing from a global talent pool, we have grown to become the largest pediatric cardiology center in the United States and one of the most specialized in the world.

Our Cardiac Intensive Care Unit is a 29-bed facility that serves as one of the largest and most sophisticated intensive care units in the country solely dedicated to cardiac care of children. We offer experience and expertise gained from treating more than 1,300 patients admitted to our unit each year and routinely care for some of the most complex cases in the world.

Our Cardiac Inpatient Unit is a 42-bed facility that provides around-the-clock care and monitoring to thousands of children - and adults - with acquired or congenital heart defects, from simple to complex cases.

Staffed by highly trained cardiologists, cardiac nurses, child life specialists, social workers, and dietitians, our goal is to make sure your child is comfortable and getting the best care possible. Our specialized training in pediatric cardiology means that we understand the unique challenges, circumstances, and intricacies of working with young people who have heart disease and other heart ailments.

Shift: Monday - Friday 8:00am - 4:30pm

This Patient Navigator will be responsible for:

  • Meets with/calls patients and families to review the implementation of care plans. Refers patients to services frequently used in a clinic and specialized services as needed. Explains hospital/community- based resources to patients and families, providing documentation.

  • Assesses patients for referrals to resources. Acts as a liaison with internal and community resources. May contact hospital and community resources to ensure access to services.

  • May assist patients and families with scheduling multidisciplinary appointments. Reviews appointment schedule and appointment instructions. Coordinates logistics for plan adherence such as appointment reminders and arranging transportation.

  • Assists patients with applying for insurance. Provides insurance information to care teams to determine eligibility for services.

  • Maintains relationships for accessing community-based resources. Provides outreach and maintains positive working relationships with providers/administrative staff (including physicians, office staff, nurses, and social services staff). Reports outcomes of referrals to referral source.

  • Documents patient care and collects required data. Tracks outcomes and supports strategic planning processes. Develops and maintains data.

  • Attends and participates in care planning meetings to prepare patients for discharge or continuing care.

To qualify, you must have:

  • A High school diploma or equivalent G.E.D. required

  • A Bachelor's Degree Human Services or a related field is preferred

  • A minimum of one year of closely-related experience

  • Experience with the clinic population or work with community resources is preferred.

  • The ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.

  • Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.

  • Fluency in Spanish may be preferred.
