The Job The purpose of this role is twofold: (1) to deliver and develop high quality behavioral healthcare for SCAN's Integrated Care team, providing leadership to the Collaborative Care model implementation, and (2) lead a team of licensed clinical social workers (LCSWs), Community Health Workers (CHW) supporting the Embrace clinical team, while additionally maintaining a clinical focus and provide direct clinical care to Embrace members through a collaborative care program. This position will be approximately 75% clinical and 25% administrative. You Will Design and implement protocols regarding behavioral health needs for SCAN's members serviced by the Integrated Care Team, including appropriate escalation protocols to address complex behavioral health needs. Identify and resolve behavioral health needs for members enrolled in plan serviced by the Integrated Care Team in collaboration with nurse practitioners, physicians, and case managers. Provide psychotherapy as allowed by license to members through referrals from team members including MD, NP, and Case Management. Develop and execute on hiring strategy to build behavioral health team as integrated care team scales to more members, to new markets, and serves new products. Onboard new behavioral health providers. Serve as an internal BH consultant for Integrated Care Delivery clinical staff to support effective, safe, and cost-conscious behavioral health plans for SCAN members enrolled in plan serviced by the Integrated Care Team. Manage team performance according to specified clinical KPIs, including referral volume, access to care, productivity, and BH clinical outcomes in order to maintain high standards of care. Provide telephonic or virtual short-term mental health treatment to members including assessment, diagnosis, treatment planning, interventions (individual and group), and discharge/aftercare planning through a collaborative care program to Embrace members residing in senior living communities, skilled nursing facilities (SNFs), and board and care facilities. Manage the execution of strategies through program design and implementation, ensuring strategic alignment with executive-level partner(s). Establish and monitor key performance indicators throughout the implementation process. Design thoughtful evaluation plans for new programs that allows for meaningful conclusions about the impact of the program on the desired outcomes. Modify programs based on outcomes. Review and improve existing behavioral health trainings for SCAN staff members and develop new ones as needed to ensure awareness of behavioral health resources for members, improve behavioral health coaching or intervention skills, and assess and manage mental health risk for SCAN members. Serve as the key point of contact for Embrace complex behavioral health cases (psychiatric inpatient or skilled nursing), collaborating with all Embrace teams to assist with care coordination. Facilitate senior leadership decision-making by analyzing strategic alternatives and developing recommendations. Actively support the achievement of SCAN's Vision and Goals. Other duties as assigned.
