Responsibilities

JOB SUMMARY

This position is responsible for evaluating and treating clients with physical and/or cognitive disabilities in order to improve client's functional independence. Services are provided in a variety of settings including nursing home, home health care, and contracted facilities. This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.This position is responsible for supporting Tabitha's philosophy of L.I.V.E: Love your job, Invite optimism, Vision success, Embrace the Mission.

ESSENTIAL FUNCTIONS

    Demonstrate commitment to Tabitha's Crew Code of conduct.

  1. Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.

  2. Performs Occupational Therapy treatment procedures or delegates to a licensed Certified Occupational Therapist Assistant based on initial and on-going client evaluation.

  3. Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.

  4. Documents the care provided, instructions given, client's progress toward goals, and changes in condition adhering to Tabitha's policies and procedures and state/federal regulations.

  5. Reports significant changes in the client's condition to the physician and nursing staff.

  6. Communicates and coordinates care and client's progress with other team members involved in the care.

  7. Designs, implements, and evaluates therapeutic exercise programs, and instructs the client's family/caregiver/Rehabilitation Aide/ Functional Maintenance Aide in carrying out the program.

  8. Consults with the attending physician and obtains a written telephone order for changes in the client's plan of care as needed.

  9. Ensures that assigned client's clinical records are complete, timely, and reflect coordination of care.

  10. Oversees and coordinates Certified Occupational Therapist Assistant and Rehabilitation Aide client care activities.

  11. Organizes and prioritizes work to complete responsibilities according to plan within established time frames to meet the client's medical and social needs.

  12. Communicates effectively with families, staff and support services to achieve client goals and meet service expectations.

  13. Provides client care in a service environment that demonstrates honesty, integrity, compassion and trust.

  14. Promote positive client/staff relations according to the mission statement of Tabitha Health Care Services.

  15. Follows all policies and procedures for Tabitha Health Care Services and Rehabilitation.

  16. Assures delivery of optimum service by reporting to work on time as scheduled, and/or providing appropriate advance notification.

  17. Participates in personal and professional growth by attending mandatory in-services and scheduled department meetings and seminars.

  18. Follows all safety and infection control policies and practices of the facility and regulatory agencies at all times.

  19. Maintains work schedule which allows for coverage in all Rehab. programs as needed.

  20. Attends and actively participates in client care conferences as indicated whenever on duty.

  21. Maintains a positive outlook toward employer and work responsibilities.

  22. Dresses and conducts self in an appropriate manner according to established procedures.

  23. Plans, coordinates, supervises, and evaluates all student interns and volunteers.

  24. Performs other duties and special projects as assigned by supervisor.

NONESSENTIAL FUNCTIONS

  1. Participates in designated department/corporate committees, (example: Safety Committee, Performance Improvement Committee).

  2. Teaches in-services relating to Rehabilitation upon request of supervisor and in coordination with Director of Education.

  3. Assists in orientation of nursing employees relating to Rehabilitation.



Qualifications

LICENSURE/CERTIFICATION

Nebraska Occupational Therapy license.

Current C.P.R. certification required.

Valid Nebraska driver's license and state required minimum auto insurance coverage required.

EDUCATION/TRAINING AND WORK EXPERIENCE

Graduate of an accredited school in Occupational Therapy required.

One year of experience preferred.

Geriatric experience preferred.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Compassion, empathy, warmth and patience are needed to inspire both trust and respect.

  • Ingenuity and imagination in adapting activities to individual needs are assets.

WORKING CONDITIONS

May encounter combative clients. Indoor environment, may be exposed to infectious diseases. Must be knowledgeable of, and follow, all safety and infection control procedures for position including Standard Precautions to prevent injury to self and others. Traveling is required when conducting home health or contract site client visits.

PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB

Must be able to walk, crouch, and kneel much of the time. Ability to lift up to 50 pounds for transferring clients and moving equipment.

EQUIPMENT OPERATION

Continuous use of: Adaptive equipment for ADL's (reachers, eating devices, dressing aides), wheelchairs/seating materials and tools, dynamometers, splints, goniometer, material handling, kitchen utensils. Frequent use of: telephone, Dictaphones Occasional use of: paraffin, hot packs.
