Position Overview

As a Residential Services Social Worker, you will carry a regular caseload at the PrairieCare Residential Treatment Center, serving youth ages 10-18. This weekend role is pivotal in facilitating groups, treatment planning, discharge planning, and care coordination. You will collaborate daily with a multidisciplinary team to advance the plan of care for our patients.

Key Responsibilities
  • Empower Young Minds: Uphold and apply your social work knowledge to support emotional and behavioral health practices.
  • Holistic Assessment: Assess, plan, implement, and evaluate psychosocial services for patients, sharing your expertise with the treatment team throughout their 30-90 day stay.
  • Lead with Purpose: Facilitate weekly multidisciplinary treatment team meetings, creating and updating master treatment plans.
  • Discharge Planning: Develop and communicate complex discharge plans with patients, caretakers, and the treatment team, ensuring a smooth transition to the next level of care.
  • Educate and Advocate: Provide psychoeducation to patients and their families about diagnoses, symptom management, and patient rights, leading care conferences and discharge meetings.
  • Build Relationships: Communicate effectively with external providers and agencies to develop strong relationships and facilitate appropriate referrals.
  • Engage and Inspire: Lead adolescent psychoeducation groups, fostering a supportive and engaging environment for learning and growth.
  • Champion Patient-Centered Care: Advocate for culturally appropriate and age-appropriate care, ensuring that every patient feels valued and understood.

Qualifications

What We're Looking For
  • Educational Background: Bachelor's degree in Social Work required; Master's degree preferred.
  • Licensure: LSW license required; LGSW or LICSW preferred.
  • Experience: At least one year of work/internship experience in social work or behavioral health care required; healthcare setting experience preferred.
  • Tech Savvy: Familiarity with electronic health records and proficiency in Microsoft Office products (Word, Teams, Outlook).
  • Team Player: Ability to react quickly and calmly in potentially volatile situations, demonstrating strong teamwork skills.
  • Effective Communicator: Excellent verbal and written communication skills to convey information clearly and efficiently.
