Description
IHA Fee-Based Nurse Practitioner/Physician Assistant Title: IHA fee-based Nurse Practitioner/Physician Assistant Reports to: Manager of Care Delivery Job Status: Fee basis pay with no guaranteed hours FLSA Status: Exempt Position Summary: The Initial Health Assessment (IHA) Fee-Based Nurse Practitioner/Physician Assistant (NP/PA) performs in-person and on occasion virtual, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members as they are assigned by specific health plans. The NP/PA will conduct thorough assessments and identify health risks to improve member outcomes. These assessments will take place within the member's personal living space according to market health plan expectations. Hours: Days, evening, and/or weekend hours required with 60-75-minute appointment time scheduling. Compensation: This role is fee-based with market rate per completed assessment. Mileage reimbursed at the current IRS rate after the first 30 miles driven for the day.
Position Essential Functions: To satisfy the needs of this position, the NP or PA must be able to meet the following essential job requirements and expectations:
- Perform comprehensive IHAs in members' homes, including medical history review, physical exams, and screenings.
- Successfully complete both the self-paced and instructor-led training components within the allotted time frame, while actively participating in periodic training sessions and meetings pertinent to the position.
- Complete a minimum of 30 appointment slots per month
- Deliver high-quality assessments that align with member satisfaction, health plan expectations, and Emcara Health quality standards.
- Identify and evaluate acute and chronic conditions and assess medication adherence.
- Document and submit chart notes accurately and efficiently within 24 hours of the visit in the Electronic Medical Record (EMR) system.
- Address all charting queries within 48 hours of receipt.
- Maintain clear and proactive communication with clinical leadership staff to ensure seamless care coordination.
- Establish and maintain a Collaborating Physician agreement with an Emcara approved physician.
- Comfortably and respectfully enter into members' homes for scheduled visits.
- Ensure reliable transportation to and from member appointments, adhering to scheduled times.
- Adhere strictly to all company policies, procedures, mission statement, values, Code of Ethical Business Conduct, and Compliance Program.
- Comply with all Federal and applicable State and local laws and Emcara Policies and Procedures regarding privacy, confidentiality, and security of health information, and other designated information.
Education and Background Check Requirements:
- Master's or Doctorate degree from an accredited NP program (FNP, AGNP, ANP) or Master's degree from an accredited PA program.
- Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
- Fee-based IHAs are required to sign an additional agreement/offer letter as part of their engagement. This agreement will outline terms related to , responsibilities, and other key conditions of their contractual relationship with the company.
Certification and Licensure Requirements:
- Current RN and NP or PA licensure in the present state of practice.
- Board certified by the American Nurses Credentialing Center (ANCC), the American Association of Nurse Practitioners (AANP), or the National Commission on Certification of Physician Assistants (NCCPA).
- Current Medicaid ID number for state
- Current Medicare PTAN Number
- Current BLS certification
- Valid unencumbered state driver's license
- Valid proof of sufficient car insurance coverage for the state that appointments are scheduled to be completed in.
- The ability to efficiently provide the internal credentialing department with all necessary documentation and requirements to obtain and maintain the appropriate licensure and certifications to practice within the state of residence.
Physical Requirements and Working Conditions:
- Ability to lift 20 lbs.
- Ability to bend or kneel for extended periods of time if needed while performing assessments.
- Additional physical demands include constant ability to remain in a stationary position, moving about inside and outside of member homes, able to communicate and exchange information with others, able to inspect information, able to perform repetitive motion with arms and fingers, and drive to and from member homes.
- Mental demands include constant ability to interpret data, problem solve, make decisions, and organize and plan.
- Work is performed in member homes and potentially exposed to adverse environmental conditions.
Competencies and Skill Requirements:
- Must be a self-motivated professional who enjoys working independently and making a meaningful impact on member health.
- Must have strong computer skills, and both knowledge and proficiency with electronic medical records.
- Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
- Ability to document assessment findings accurately and thoroughly.
- Ability to communicate effectively through verbal and written communication to members and the clinical team.
- Demonstrate empathy and respect for members while completing in-home assessments
- Maintain member, team member and employer confidentiality while remaining in compliance with all HIPAA regulations.
- Ability to make critical decisions while following Emcara Health policies and procedures.
- Ability to adapt easily to changing conditions and work responsibilities.
- An ability to display a positive attitude and maintain a collaborative relationship with health plan members and the clinical team.
- Ability to proactively anticipate and address/resolve member, provider, and health plan issues
- Demonstrated clinical skills necessary to perform in-home assessments
Qualifications
Preferred Qualifications:
- Home health experience
- 1 year of experience conducting in-home assessments or working with Medicare/Medicaid populations.
- Bilingual proficiency
