**Available to regular status employees who are scheduled to work a minimum of 24 hours. Southcoast Health is an Equal Opportunity Employer. Compensation: Pay rate will be determined based on level of experience. Responsibilities Under general supervision of the Director, Telehealth and Transitional Care or designee, perform duties to support and in support of the Tele monitoring program in an effort to reduce re-hospitalizations and provide proactive symptom management while adhering to best practices. Duties include, but are not limited to: address data packets that are sent via the telehealth system with patients, field staff, physicians and clinic staff; triage phone calls and provide patient education as it relates to data collected via telephone and video as needed; troubleshoot equipment and make home visits as needed. Implement and continue to grow programs to reduce re hospitalizations amongst high risk patients. Qualifications
