Description

JOB PURPOSE: The goal of the Case Management Pilot Program (CMPP) is to provide supportive case management services to assist non-citizens in addressing barriers to remaining compliant with Department of Homeland Security requirements. The CMPP Case Manager is responsible for delivering case management services to participants in the selected pilot site of (Miami / San Francisco). The Case Manager will be responsible for working with clients to address their basic needs; connecting them to community partners through referrals; and addressing barriers clients may encounter to attending their legal proceedings in order to ensure their compliance with court requirements.

Reporting to the CMPP Program Supervisor, this position will be a dedicated team member of the CMPP and work collaboratively with all other CMPP team members to ensure program success. Working directly with clients to address their case management needs, and working with other community partners to connect clients to needed services, will be key functions of this position.

The CMPP Case Manager will also be responsible for working with the larger CMPP team to ensure that the program meets goals for key program metrics, including case acceptance and client outcomes.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Manage a caseload of active and maintenance clients to address basic needs and barriers to court attendance.
  • Work with clients to identify barriers to accessing services, and addressing those barriers directly with service providers.
  • Maintain contact with clients for up to 9 months from case acceptance to ensure attendance at court hearings.
  • Attend court hearings with clients as available to ensure client attendance.
  • Attend periodic team meetings and other community events as required.
  • Maintain necessary data entry and documentation into the CMPP client database to ensure accurate record keeping.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's level education preferred in a human-services related field; bachelor's level education may be substituted for another education level and at least 3 years' experience in case management roles.

EXPERIENCE: At least one years' experience in case management roles preferred, with at least 3 years' experience required for candidates with less than a bachelor's level education.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. Must be able to report to CMPP office a minimum of 1x per week, or more often as business needs dictate. Candidate may be permitted to work remotely from home the remainder of business hours at the discretion of Endeavors leadership.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. This includes, but is not limited to: EOIR court locations, community partner locations, and Endeavors corporate locations. Bilingual in English and Spanish is required.
