THE POSITION

DESCRIPTION OF WORK

As an Assessor, you will be tasked with providing consumers, families, and caregivers with information and directions related to the process of obtaining Waiver Services for the community, nursing facility (NF) placement, and/or personal care home (PCH) placement. This position is responsible for linking consumers, family members, and caregivers to available community services and resources throughout Lebanon County to ensure continuity of care. Work also includes reviewing forms for completion and accuracy; completing assessments to determine eligibility; performing regular desk reviews; as well as attending appeals and hearings as needed. You will have the opportunity to consult with your supervisor to ensure consumers are appropriately placed for their current level of needs.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Successful completion of the County Social Casework Intern program; or
  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

Other Requirements:

  • You must be able to perform essential job functions.
