YouthBuild Mentorship Coordinator - Case Manager

Community Teamwork is seeking a YouthBuild Mentorship Coordinator - Case Manager, who will be responsible for ensuring the successful operation of the YouthBuild mentorship program, linkages to internships, and overseeing mentoring responsibilities. Working in collaboration with the Program Manager, perform marketing and outreach of the YouthBuild Program within the community. Provide education, occupational training, and placement services for YouthBuild students; provide guidance and encouragement to students in the development of leadership skills to enable students to develop a plan for self-sufficiency.

Responsibilities include:



  • Promote community development and relationship building for CTI's YouthBuild Program within the community
  • Includes program marketing and outreach, as well as seeking, establishing and maintaining relationship partnership businesses
  • Develop and implement training for the mentor curriculum and programing
  • Work with community partners to develop internship and mentoring opportunities for students
  • Prepare required reports on a monthly, quarterly and annual basis in a timely manner
  • Responsible for receiving, assigning, and monitoring new referrals
  • Develop and maintain positive relationships with other public/private sector individuals directly or indirectly connected with the mentorship field or those having interests in the YouthBuild program
  • Establish, evaluate and monitor goals for clients linked to measurable outcomes. Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services
  • Maintain the structure and policies of the program through orientation of participants by reviewing program rules, policies, and case management visits
  • Maintain accurate case records by documenting all services rendered, including up to date case notes, telephone contact logs and referrals. Enter required data into the agency databases with reporting as required


Qualifications include:



  • High School Diploma or equivalent required, Bachelor's Degree preferred
  • Minimum of one year of assessment, advocacy and case management work in a Human Service field required
  • Ability to work effectively with people from a diverse range of backgrounds, cultures, and identities, including underserved communities
  • Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals
  • Demonstrated knowledge of community resources available to low-income individuals and families
  • Excellent customer service, organizational, oral and written communication skills
  • Ability to manage multiple priorities and work in a fast-paced environment
  • Proficient computer skills required in basic computer software and data collection systems
  • Excellent crisis management skills
  • Ability to maintain accurate records and confidentiality of information
  • Excellent attention to detail
  • Bilingual (English/Spanish, English/Khmer, English/Portuguese) preferred
