Job Summary and Responsibilities Acentra Health is looking for a Clinical Assessor to join our growing team. Job Summary: As a Clinical Assessor at Acentra Health, you will play a vital role in supporting your community by conducting comprehensive, needs-based eligibility assessments for patients applying for Medicaid-funded personal care services. These evaluations, completed in settings such as nursing homes, hospitals, and community-based locations, are essential in determining the appropriate level of care and ensuring patients receive the support they need to enhance their well-being and quality of life. In this role, you will provide critical preadmission screening and resident review (PASRR) Level II evaluations for individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC). Your work will directly influence care planning, placement decisions, and the facilitation of next-level care for those in need.
