Position Summary

Oncology Nurse Coordinator works closely with assigned physicians and nurse navigators to facilitate the care of cancer patients undergoing active cancer treatment(s) at Stony Brook Medicine.

The primary purpose of the Oncology Nurse Coordinator is to:



  • Establish and coordinate a care map for patients undergoing active cancer treatment.
  • Act as a liaison and support system that enable patients to receive treatment in an organized coordinated manner.
  • Serve as a patient resource during the patient's treatment.
  • Work in collaboration with attending physicians and staff nurses, ensuring high quality care while receiving complex cancer treatment.


Duties of the Oncology Nurse Coordinator include, but are not limited to the following:



  • Serves as a liaison for assigned physician practices.
  • Formulates and updates the plan of care that addresses: educational, financial, social work, etc. needs.
  • Perform appropriates reassessments to include psychological, physical, environmental and functional needs and makes referrals to appropriate resource.
  • Facilitates communication between the attending staff, residents, fellows, nurses and secretarial support staff.
  • Follow patients through their surgical or medical oncology course of care.
  • Provides the patient and family with education regarding their disease specific plan of care.
  • Assist with the development and or update of policies and procedures related to coordination.
  • In coordination with nurse navigators and the disease management team, assist in the completion of cancer survivorship care plans.
  • Provide patient/family education and explanations about the course of treatment and care.
  • Participates in and contributes expertise to activities and functions that strengthen the cancer program including, but not limited to continuous program and quality improvement.
  • Coordinate and schedule treatment and clinic visits with clinic staff ensuring timely care is received.
  • Facilitate the flow of clinic and treatments during assigned physician sessions. Orients new patients to the service.
  • Perform ongoing assessments and communicates changes to physician and nursing staff as needed.
  • Receive and return patient inquiries regarding symptom management and their plan of care.
  • Perform follow up telephone calls to patients after their first treatment.
  • Create and maintain a binder or electronic document on all active treatment patients.
  • Communicate problems or barriers to direct supervisor for problem solving.
  • Ensure chemotherapy orders are available to infusion staff 2 days prior to the infusion visit.
  • Actively seeks and participates in educational opportunities, both formal and informal, to achieve and maintain knowledge and skill related to job responsibilities.


Qualifications

Required:



  • NYS RN license.
  • Three (3) years acute nursing experience, with minimum 2 years in oncology.
  • Bachelor's degree in Nursing or current enrollment in BSN program (proof must be submitted).


Preferred:



  • Certified Oncology Nurse.
  • Master's Degree in Nursing
  • Previous experience in case management.
  • Experience with the medical oncology patient population (breast, lung, etc.)

Prior to start date, the selected candidate must meet the following requirements:



  1. Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services*
  2. Complete electronic reference check with a minimum of three (3) professional references.
  3. Successfully complete a 4 panel drug screen*
  4. Meet Regulatory Requirements for pre employment screenings.
  5. Provide a copy of any required New York State license(s)/certificate(s).


Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.

*The hiring department will be responsible for any fee incurred for examination.

Anticipated Pay Range:

The salary range (or hiring range) for this position is $ 88000 - $ 125466 / year.

The above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate's validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.

Some positions offer annual supplemental pay such as:



  • Location pay for UUP, CSEA & PEF full-time positions ($3400)
