Responsibilities
- Provide educational materials to patients and families.
- Collaborate with LCSW or Nurse Navigators for patient assistance.
- Conduct survivorship and care planning visits; make referrals (excluding breast cancer).
- Engage in community outreach and cancer prevention education.
- Make weekly calls to patients with cancer-related pain using Ontada reports.
- Complete forms and health screens as directed by the Senior Quality Programs Manager.
- Document patient interactions in the EMR accurately and promptly.
- Assist with the Angel Trust application process.
- Refer patients to services for housing, transportation, and more.
- Maintain a community resource list.
- Help resolve complex issues with coworkers.
- Integrate Ontada Reports into daily practice workflows.
- Develop health equity plans for quality improvement.
- Address health disparities with evidence-based strategies.
- Identify financial resources with Patient Financial Counselor or LCSW.
Qualifications
- BSN-RN with at least five years of oncology experience.
- Oncology Certification (OCN) and Nurse Navigator Certification preferred.
- Proven leadership and interpersonal skills with prioritizing and critical thinking abilities.
- Strong organizational skills and a commitment to continuous quality improvement and health equity.

- Expertise in nursing processes: assessment, diagnosis, care planning, implementation, and evaluation.
- Skilled in addressing health disparities and patient navigation.

- Outstanding verbal, written communication, and customer service skills.
- In-depth knowledge of oncology care, outpatient workflows, and community resources.
- Proficient in MS Office and internet research.
Physical Demands: Requires full range of body motion, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extensive periods. Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range. Must lift and assist patients as needed. Work Environment: Exposure to communicable diseases, toxic substances, ionizing radiation, medical preparations, and other conditions common to an oncology/hematology clinic environment may occur.
