Key Responsibilities
  1. Care Coordination:

    • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
    • Facilitate the seamless transition of care across various settings, such as hospitals, outpatient facilities, rehabilitation centers, and home care.
    • Patient Advocacy:

      • Serve as a liaison between patients, families, and healthcare teams to address needs, concerns, and goals of care.
      • Advocate for patients to ensure they have access to necessary resources, services, and support.
      • Assessment and Planning:

        • Conduct comprehensive assessments of patients' medical, psychological, and social needs.
        • Identify barriers to care and develop strategies to overcome them.
        • Resource Management:

          • Collaborate with insurance companies and payers to ensure coverage for required treatments and services.
          • Assist patients and families in navigating healthcare systems and community resources.
          • Quality Assurance:

            • Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
            • Maintain compliance with organizational policies, healthcare regulations, and industry best practices.
            • Documentation and Reporting:

              • Maintain accurate, thorough, and up-to-date patient records in compliance with legal and organizational standards.
              • Provide detailed reports on case management activities and outcomes to relevant stakeholders.
              • Collaboration:

                • Work closely with multidisciplinary teams, including physicians, nurses, social workers, therapists, and community organizations, to deliver coordinated care.
                • Participate in case review meetings and contribute to care improvement initiatives.
                Qualifications

                Education and Certification:

                • Bachelor's degree required
