HIV Clinical Social Worker, LCSW or Licensed Professional Counselor, LPC

The Ryan White HIV Clinic seeks an HIV Clinical Social Worker, LCSW or Licensed Professional Counselor, LPC. The Clinical Social Worker is responsible for conducting face to face sessions with patients in order to complete thorough assessments, develop treatment plans, and provide supportive counseling, crisis intervention and brief therapy. Provides clinical services to patients in the Infectious Disease clinic. Responsible for serving populations with a variety of clinical needs particularly psychological stress related to health impairments, chronic illness, trauma, as well as substance abuse. Works with victims of intimate partner violence, abuse or exploitation, and facilitate the identification and implementation of healthy boundaries and effective communication techniques. Works with patients and team members in an effort to maximize patients' potential for the healthiest coping responses to their challenging situation. Based on overall assessment and summary of patients and family needs, makes appropriate community referrals for follow up as needed. Supervises those seeking to get their LCSW License. Participates in clinical training and supervision as needed.

This Clinical Social Worker will provide care in a busy HIV clinic, working closely with Medicine and Psychiatry Attendings, APPs, Residents, Fellows, Case management team members, Pharmacists, RN Coordinators and clinic team members. You should have exceptional communication skills, work well with a large diverse team and have an interest in HIV. The UVA Health Ryan White HIV Clinic has a large multidisciplinary team who care for approximately 1,000 patients living with HIV. This HIV clinic is regularly recognized by the Virginia Department of Health and the Health Resources and Services Administration for best practices and outstanding clinical outcomes.

Key Performance Area: Patient Care

1. Provides direct social work service to patients by providing assistance with social and practical environmental problems.

• Screens patients to identify psychosocial needs/acuity and opportunity for SW intervention based on case finding and patient needs. Accurately evaluates social and family situation to determine social work treatment plan.

• Provides assistance with all aspects of social and practical environmental problems. Conducts groups with patient and family populations as clinical situation allows.

2. Provides emotional support to patients with anxiety and stress precipitated by illness and hospitalization. Conducts interventions based on needs identified during initial assessment.

• Assists all identified patients with emotional support in accordance with acceptable social work practice, Provides counseling to patients with issues related to institutional and specialized care in a timely and efficient manner.

• Crisis Interventions: Supports care team during medical crisis, adult & child protection/guardianship, adolescent pregnancy, substance dependency/abuse, suicide attempt, trauma/victim of violence, and abuse and/or neglect.

• Psychosocial Assessments: Completes psychosocial assessment for patients based on initial screening, case finding and referrals. Documents all assessments and progress notes in EPIC according to departmental guidelines. Communicates with Case Manager (CM) and care team based on the outcomes of the assessment.

• Routine Interventions: Patient advocacy, continuum of care, and referrals to community resources, occupational/educational counseling, new diagnosis and/or adjustment to illness counseling, grief and bereavement.

• Leads group and/or participates in group and or group supervision.

3. Maintains acceptable working knowledge of resources needed to provide patients with continuity of care.

• Regularly updates knowledge base on subjects regarding State (Medicaid), Federal (Medicare) requirements and the regulations/resources needed to provide patient with continuity of care.

• Accurately documents and communicates the precise needs of patients to community resources in an efficient and timely manner.

4. Participates in in-service and staff development activities by attendance, serving on planning committees and/or offering topics for pertinent educational programs.

• Actively leads staff discussions on topics which relate directly to particular service or social work theory as scheduled by department.

• Participates in the preparation of staff LCSW licensure via LCSW supervision or professional development.

5. Documents all activities relevant to casework, patient care and follow-up in accordance with UVA and Social Work Policy, Procedure and Guidelines and regulatory requirements.

• Records High-risk screening or referrals in the medical record within 24 hours of receipt, Records all activities related to EPIC documentation and patient progression accurately and timely in the electronic medical record.

• Documents casework activity for each episode of outpatient intervention within 72 hours.

• Demonstrates/role models time management and organizational skills on a routine basis.

6. Participates in education/research activities.

• Participates and assists in departmental studies and projects as assigned.

• Provides peer supervision to newly hired social workers.

• Provides education to social work students, medical residents, staff, nursing students and other allied health students and interns.

• Participates/attends workshops to enhance social work skills and knowledge and for licensure purposes.

7. Provides individual counseling/problem solving sessions as needed.

• Communicates interventions to the patient treatment and care team.

• Documents interventions and patient's response to the intervention in their medical record.

Training and other Requirements:

• VAMAP requirements and applications.

• Medicaid, Medicare, SSI and SSDI requirements.

• HIV Case Management Standards.

• HIV 101: including diseases process, treatment, testing.

• Legal issues such as confidentiality, counseling/referral and prevention.

• Cultural Competency.

• RW Eligibility requirements.

• Motivational Interviewing training.

• Participates in staff trainings and in-services at VDH.

• Participates in opportunities offered by the MidAtlantic AIDS Education and Training Center and Virginia HIV/AIDS Resource and Consultation Center.

• Participate in educational training provided by local agencies in regarding to HIV care or services needed by clients.

• 12 hours of continuing education is required for LCSW working with RW programs.

MINIMUM REQUIREMENTS:

Education: Master's degree in Social Work from a CSWE accredited social work program required

Experience: Minimum of five years' experience in the social work field required; experience in health care settings is preferred. Therapy experience will also be considered.

Experience: A minimum of two years of experience in a healthcare setting or social agency, which could include a social work internship.
Licensure: Licensed to practice as a Social Worker in the Commonwealth of Virginia (LCSW) or (LPC).

PHYSICAL DEMANDS
Job requires sitting/standing for prolonged periods, frequently bending/stooping/traveling (distance: 1 - 2 miles/day), climbing (ladder, steps) and reaching (overhead, extensive, repetitive). Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs.
