Are you looking for an exciting opportunity to be part of a growing Community Health Worker team?

Presbyterian is now hiring a Community Health Worker - Level 1 for Northern New Mexico. The role is located in an outpatient clinic setting, and can be based out of Santa Fe, Rio Arriba or San Miguel counties.

This position will provide patients with basic health education to support chronic disease prevention and management, referrals to community-based organizations, and coordinate various community health programs, including produce prescription programs and health promotion classes. We are looking for candidates who have experience working in a clinical environment and/or health promotion.

Responsibilities

  • Participate in the planning and implementing, the plan of care as developed by Care Manager
  • Report changes in patient condition as reported by patient to Care Manager and/or Primary Care Provider as appropriate. Support patient and family teaching.
  • Maintain a safe environment for patients, families and visitors.
  • Keep current with community health care worker standards, PHS philosophy, and PHS programs in order to maintain quality.
  • Maintains education development through participation in education programs current literature, inservice meetings and workshops
  • Performs other functions as required.
  • Interacts effectively with customers, providing instructions and information, both written and verbal as appropriate
  • Participates in local, regional or state community health care worker events/meetings
