Overview Now hiring a Community Health Worker! Are you looking for an exciting opportunity to be part of a growing Community Health Worker team? Presbyterian is now hiring a Community Health Worker - Level 1 for Northern New Mexico. The role is located in an outpatient clinic setting, and can be based out of Santa Fe, Rio Arriba or San Miguel counties. This position will provide patients with basic health education to support chronic disease prevention and management, referrals to community-based organizations, and coordinate various community health programs, including produce prescription programs and health promotion classes. We are looking for candidates who have experience working in a clinical environment and/or health promotion. How you belong matters here.
