The Ryan White Non-Medical Case Manager will work closely with clients living with HIV/AIDS who have multiple psychosocial and/or health related needs. The Non-Medical Case Manager will work closely with clients living with HIV/AIDS who have multiple psychosocial and/or health related needs. Non-Medical Case Management services is a strength- based approach to service that includes treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, social support and any other needed service. The goals of Non-Medical Case Management is to insure that clients with HIV/AIDS have timely access to comprehensive medical care and social services; prevent disease transmission and delay of HIV progression and to promote and support client independence and self-sufficiency. The Non-Medical Case Manager will be supervised by the Medical Case Manager Supervisor. Prior working experience with the HIV/AIDS community is preferred. Key Performance Area: Screening
Works to Schedule patients for appointments and supports the patient check out process after their medical appointments. Works in collaboration with the Medical Providers in the ID clinic to identify clients at intake that are in need of Medical Case Management. Complete patient screening to determine eligibility for medical case management services. Collaborate and Consult with Clinical Staff to identify current clients who are in need of medical case management services. Respond within 24 hours to requests for case management services. Provide feedback to patient's provider care team in the ID clinic as needed.
Key Performance Area: Orientation and Assessment
Determine client eligibility for various RW funded services (including MCM and the Virginia ADAP and other community resources). Provide orientation to the clinic operations for new clients. Obtain proper documentation for proof of HIV status, residency information, proof of income, and uninsured or underinsured status as part of the RW Part B for initial and recertification eligibility determinations. Conduct client intake interview(s) and complete intake application and all required forms. Refer patient to MCM to schedule MCM Assessment appointment. Perform the six-month eligibility review and recertification and obtain necessary documents as identified in the NMCM standards. Maintain documentation and program notes in the client records according to Virginia Department of Health (VDH) requirements and NMCM standards. Complete client data entry into VDH's & HRSA client-level database. Coordinate with Outreach, Patient/Peer Navigation, and VDH Care Coordination staff to facilitate access to care or referral to re-engage out-of-care clients. Coordinate eligibility and intake services with community agencies, hospitals, and physician practices to assist clients to access services. Maintain current information on all frequently used community resources, as needed, and provide to clients who need identification of new resources.
Key Performance Area: Continued Contact
Face to face interaction with client at least every 90 days. If patient is in need of 90-day check-ins, then they need to be in Medical Case Management Document treatment adherence activities in client's file; these include keeping medical appointments, taking prescribed medication, refilling prescriptions, etc. This as well is MCM services; NMCM only assist with pharmacy issues, i.e. Ramsell card & copay issues. NMCM assist with food cards, transportation, eligibility, VAMAP/PFS completion, Case Management referrals (NMCM assessment done during intake or if client proceeds to have same issue 3x or more) In addition to the above job responsibilities, other duties may be assigned.
Training and other Requirements:
ADAP requirements and applications. Medicaid, Medicare, SSI and SSDI requirements. HIV Case Management Standards. HIV 101: including diseases process, treatment, testing. Legal issues such as confidentiality, counseling/referral and prevention. Cultural Competency. RW Eligibility requirements. Participates in staff trainings and in-services at VDH. Participates in opportunities offered by the VCU HIV/AIDS Center. Participate in educational training provided by local agencies in regarding to HIV care or services needed by clients. 5 hours of continuing education is required for Non-Medical Case Managers.
MINIMUM REQUIREMENTS: Education: HS Diploma Experience: 1yr experience working with persons living with HIV or related experience of two years, regardless of academic preparation. Licensure: None
