Residential Discharge Specialist

Responsibilities:

As a Residential Discharge Planner you will be responsible for ensuring that appropriate discharge plans are in place prior to residential discharge and appropriate clinical services are coordinated for the member, to support the successful transition in the community setting and reduce the risk of readmission.

In addition you will educate clients and support staff on housing options, treatment options, and community resources, available to clients when leaving residential treatment. (housing, group counseling, individual counseling, PCP, behavioral health medication management, 12 step meetings, supported education, employment services, peer support certification, psychosocial rehab, etc.).

Qualifications:

* A High School Diploma/GED and three years of relevant experience in a behavioral health setting; OR

* An Associate's degree and two years of relevant experience in a behavioral health setting; OR

* A bachelor's degree in the behavioral health field; OR

* A non-behavioral health field bachelor's degree and one year of relevant experience in a behavioral health setting

* Must be at least 21 years of age.

* Background check.

* Minimum typing speed of 25 wpm.

* Some assignments require an Arizona Fingerprint Clearance Card

* Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
