Worksite Wellness is looking for a Per Diem LPN OR CMA ( As Needed) to join our team!

Position Summary:

The Licensed Practical Nurse (LPN) provides basic nursing care (as defined in the Board of Nursing Standards of Practice) and delivers physician-delegated aspects of care to individual out-patients. He/she performs under the general supervision of the physician staffing clinic.


Education and Training Required:

a-Graduate from an accredited school for licensed practical nurses.

a-Current licensure as a practical nurse in the state of Wisconsin.

a-Certification of completion of a pharmacology course.

a-BLS certification required.

a-Must be able to deal with multiple priorities in a calm way.

a-Requires decision-making and problem-solving skills.


Physical Requirements:

a-Physical Demands:

aFrequently sit while performing the tasks of the role (scheduling, developing & reviewing protocols/procedures, job descriptions, etc.).aOccasionally walk and stand.

aOccasionally lift up to 50 pounds while transferring and positioning patients and maneuvering equipment while using gait belts, transfer devices, etc.

aOccasionally bend to the floor, squat, kneel, crouch and reach overhead to assist with patient care or retrieve equipment or supplies.

aOccasionally handle objects, smell, feel, control knobs, switches and levers.

a-Sight:

aOccasionally use sight capacities, including spatial, peripheral, colors & accommodation and depth perception.aFrequently use near and far vision while reviewing patient records, computer screens, charts, hemoccult results, gauges on patient equipment, monitors, etc.

a-Cognition/Communication:

aFrequently utilizes simple reading skills, normal conversations.

aOccasional exposure to comprehensive speech, both simple and complex writing skills, basic and complex math.aFrequently is exposed to low volume speech with patients and telephone conversations with other members of the healthcare team, physicians and family members.

a-Environmental Conditions:

aConstant exposure to an inside, patient environment with occasional unpleasant odors or noises.aOccasional exposure to low light or glare conditions.

a-Potential Hazards:

aOccasional exposure to infectious blood/body fluids and diseases requiring infection control practices, red infectious waste.

aOccasional exposure to needles/syringes/sharps during medication administration, assistance with treatments, and invasive procedures.

a-Psychological, Situational, Functional Factors:

aFrequent problem solving, analyzing, use of judgment and critical thinking in regards to lab values, changes of patient condition, environmental factors, flexibility.

aOccasionally able to rotate shifts, work overtime and be on call.aFrequently able to work alone, take initiative, work closely and motivate others.

aContinuously plan and direct patient care, manage conflict ad/or difficult situations.

aOccasionally prioritize work flow while maintaining speed, accuracy and consistency.
