Job Details

The Community Health Worker (CHW) is a liaison between the community members/patient/families and community service agencies. The CHW will assist individuals and communities by addressing barriers to behavior change with an awareness of the impact of social determinants of health. The CHW will assist community members/patients and their families to navigate and access community resources and services and provide social support and informal counseling, and advocate for individual and community health needs.

Job Description

Essential Functions:

COMMUNITY BUILDING/COMMUNITY ENGAGEMENT

  • Understands the importance of practicing cultural humility when interacting with community members and others
  • Demonstrates clear communication and positive interpersonal skills
  • Establishes trusting relationships with community members and their families/caregivers while providing general support and encouragement
  • Fosters and sustains relationships with community stakeholders and communities, including Jefferson staff
  • Continuously expands knowledge and understanding of community resources, services, and programs and social determinants of health
  • Attends community events and meet with community organizations (i.e., libraries, business owners, block captains, etc.)
  • Assists and supports the planning and implementation of culturally and linguistically appropriate community outreach events and educational activities

CARE COORDINATION/WORKING WITH COMMUNITY MEMBERS

  • Screens (SDoH, benefits, etc.) and refers individuals for appropriate health and social services to address individual and family needs
  • Assists and advocates patients and families in identifying and prioritizing needs, navigating systems, and accessing services
  • Provides referrals for community agencies as appropriate
  • Provides ongoing follow-up, basic motivational interviewing, and goal setting with patients/families
  • Provides education to patients and families related to health and wellness, community resources, and self-care activities
  • Manages caseload of patients/community members

PROGRAM SUPPORT

  • Provides clear and concise written documentation of patient/family and community interactions
  • Enters program data into appropriate databases and prepares reports and program summaries
  • Attends trainings/programs to further develop skills and knowledge
  • Supports program implementation as requested including but not limited to development of program materials, policies and procedures, and research and evaluation activities
  • Works collaboratively and effectively within a team
  • Builds and maintains positive working relationships with the community members, community leaders, providers, care coordinators, social workers, office staff, and others
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson

Required Education and Experience:

Education:

  • High school diploma or GED
  • Community Health Worker Training
  • Certified Community Health Worker preferred
  • One year experience preferred
