With a commitment to patient- and family-centered care, the Certified Wound Continence and Ostomy Nurse uses the nursing process in ways that values the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health. The Certified Wound Continence and Ostomy Nurse is a registered nurse with certification in this specialty (CWON or CWOCN). He/she functions independently and collaboratively with the multidisciplinary team to provide specialized care for patients with wound, ostomy, and continence concerns in the acute care setting.

The Certified Wound Continence and Ostomy Nurse influences patient care both directly, through the provision of hands-on care, and indirectly, by serving as an educator, consultant, researcher, or administrator guiding the provision of optimal patient care to patients with wound, ostomy, or continence concerns. The Certified Wound Continence and Ostomy Nurse participates in patient care assessment, planning, implementation, and evaluation for inpatients.

Job Description

  • Interacts with co-workers, patients, their families/designated care partner(s), and other staff consistent with the values of Jefferson.
  • Integrates evidence based practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal health care.
  • Partners with patients, their family/designated care partner(s) in identifying learning needs and priorities and provides education in ways that is useful, understandable and affirming.
  • Participates in patient care assessment, planning, implementation and evaluation for outpatients with wound and/or ostomy care needs. In this role, teaching and support is given to the patient and/or caregiver.
  • Provides care for stomas, complex wounds, fistulae, pressure ulcers, and fecal and urinary incontinence.
  • The CWON or CWOCN Nurse provides specialized knowledge and skill to assist physicians and nursing staff with the management and teaching of patients with complex skin care needs, and guidance for selection of related product formulary.
  • Provides specialized knowledge and skill to assist physicians and nursing staff with the management and teaching of patients with ostomies or patients with complex skin care needs.
  • Acquires and maintains current knowledge in nursing practice to deliver quality patient care.
  • The Certified Wound Continence and Ostomy Nurse provides mentoring for hospital staff in relation to these issues and assists in setting guidelines for patient care. These services and processes are performed in accordance with standards of care and hospital guidelines.
  • Leads, coordinates and carries out a wound care shared governance committee which includes collection and reporting of data such as incidence, prevalence, and pressure injury prevention interventions.
  • Conducts product review related to wound and ostomy care collecting recommendations, evaluations and development of protocols for utilization.
