Licensed Practical Nurse - LPN - Continuous Care

Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team.

8am - 8pm. Three12 hour shifts with weekend requirement - days to be determined. Coverage area to include Manatee, Charlotte, De Soto, and Sarasota County.

What you do will do:

Primary tasks, duties, and responsibilities:

Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.

In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Crisis Care nurses are cross-trained to Hospice Houses and Triage to assist with patient care needs when cases are pending.

Requirements:

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
