What You'll Do: Provide personalized community health support to high-risk patients facing medical and psychosocial challenges. Educate patients on how to take their blood pressure properly and support them in improving their blood pressure control. Motivate and coach patients in achieving their health goals through, provider practice visits, community visits, and follow-up calls. Address Social Determinants of Health (SDOH) by connecting patients to crucial resources such as housing assistance, food programs, and transportation. Document patient encounters and collaborate with care teams to ensure comprehensive patient care and advocacy. Attend ongoing training and education to enhance service delivery and stay up to date on best practices. What We're Looking For: High School Diploma or GED required. A valid MA driver's license to meet patients where they are. A passion for helping others, strong emotional intelligence, and the ability to build trust with patients and care teams. A strong connection to the community - local residents who understand available resources are highly preferred. Bilingual skills in Spanish or Haitian Creole are highly preferred.
