SUMMARY:The Patient Safety Specialist reports to the system Director of Patient Safety with indirect reporting to the assigned hospital entity Directors of Quality. Under general supervision and within Brown Health established policies and procedures the Patient Safety Specialist oversees and manages the safety event reporting workflows and processes ensuring for timely review of all reported safety events through the process of screening and triage consultation documentation and closure. The Patient Safety Specialist screens events performs a preliminary investigation of safety event data and consults appropriate leaders for further investigation. The Patient Safety Specialist supports tracking and trending of reported events to identify performance improvement opportunities. This role provides critical collaboration with Risk Management and other leaders in the investigation of safety events. The Specialist may consult senior and unit leadership Risk Management and others as indicated and based on event type harm level and other factors. The Specialist conducts and facilitates Critical Event Huddles RCAs and other methods of causal analysis. As part of the Safety event closure process the Patient Safety Specialist ensures that Safety event data is submitted to the Patient Safety Organization (PSO) via the event reporting system. REQUIRED QUALIFICATIONS: Baccalaureate degree in nursing health services administration public health or related field required. Advanced degree in related field preferred. Certified Professionalin PatientSafety (CPPS) preferred. Certified RCA facilitator preferred. Strong verbal written and presentation skills required.Six Sigma/Lean Green Belt or Black Belt certification desired or completed within 24 months of accepting position. PREFERRED QUALIFICATIONS: Three years of relevant experience in Quality Patient Safety or Risk Management. Experience in facilitating FMEA's and use of Root Cause Analysis tools facilitation of small and large groups project teams performance improvement systems and/or process improvement methods and standards. Project management experience preferably in a large complex healthcare organization is desired. Knowledge of the adverse event reporting systems. PRIMARY DUTIES AND RESPONSIBILITIES: Ensures timely screening and monitoring of all safety reported events. Escalates concerns trends high harm or serious near miss events to leadership in a timely manner. Provides oversight for screening monitoring and maintenance of the event reporting system ensuring accuracy of event reporting data. Assigns harm levels documents investigatory findings among other functions associated with the event management process. Collaborates Risk Management Nursing medical staff Pharmacy and other departments across the system to ensure optimal workflow and accurate reporting of event data. Facilitates Critical Event Huddles casual analyses and other methods and processes related to safety event review investigation and management. Communicates importance of safety event reporting safety culture identification of unsafe systems and processes (supports Great Catch). Provides teaching and education on safety science high reliability and communications expectations on event reporting processes to front line staff key stakeholders and leaders across the entities and system. Identifies best practices. Monitors and make suggestions to improve best practices and their integration into existing systems. Collaborates with staff and physicians across the system. Leads and facilitates RCAs ACAs and FMEAs across the Brown Health system. Identifies issues through failure analysis; assists improvement teams in understanding issues and improvement opportunities related to improving safer systems of care. Consults providers in the Peer Review process. Works collaboratively with Risk and Quality to ensure timely review and provider consultation. Supports the development of action plans in response to reported events and causal analyses. Demonstrates technical expertise in management of the event reporting system. Knowledge of analytics and reporting. Proficient in Excel. Reviews appropriate literature to support investigations policy and process improvement opportunities. Prepares reports patient safety reports for committees leaders and other affiliates and medical staff to drive safety initiatives. Performs other duties as assigned. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Work is performed in a typical office setting requiring extensive periods of sitting at a computer orlap top with arms extended as well as walking and standing. INDEPENDENT ACTION: Performs independently within the department policies and procedures; refers specific problems to the Director where clarification of departmental policies and procedures or workload may be required. Must be self-directed and able to stay abreast of changing regulatory standards and safety culture principles. SUPERVISORY RESPONSIBILITY: None.
