Responsibilities
  • Provide Comprehensive Home Health Care - Conducts in-home patient visits, performs assessments, develops and updates individualized care plans, and delivers high-quality nursing care as directed by physicians to ensure patient safety and independence.

  • Coordinate Interdisciplinary Care - Collaborates with physicians, caregivers, and the healthcare team to optimize patient outcomes, prevent hospitalizations, and ensure effective communication and care plan adjustments.

  • Maintain Accurate Documentation & Compliance - Ensures timely and accurate clinical documentation, adheres to home health regulations, and participates in quality assurance, performance improvement, and professional development activities.


Qualifications
  • Graduate of State Board approved program for Registered Nurses
  • One year nursing experience caring for similar patient population as to be assigned.
  • Valid license as RN in state(s) where providing care.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
