Description

JOB SUMMARY: Under close supervision, responsible for assisting with Individual and/or group
services, which include assessment, coordinating, planning, advocacy, linkage, referral,
education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way.
Provide point of contact and work in partnership with others as part of a multidisciplinary team to
provide a set of social service functions that helps clients access the resources they need to recovery.
Services are provided in the office or in the community based on selected program.

DUTIES & RESPONSIBILITIES:
1. Assist with case management and rehabilitation services using a strengths based, welcoming,
recovery oriented, client driven approach for an assigned group of clients
* Observe the delivery of individualized recovery and resilience-oriented care that is clearly
directed by those receiving services, including those receiving services in multiple settings
or from multiple systems
* Assist in providing education for clients and their families on recovery and prevention to
empower them toward full participation in directing their own care or assisting family
members as desired to assist with planning and directing care.
* Notify supervisors to intervene when a client is at risk for harm to self or other,
hospitalization, or involvement with law enforcement. Respond to crises and provide
intervention.
* Monitor support to a client throughout the course of his/her treatment.
* Assure continuity of services by telephone calls, written communication and or outreach to
clients who missed appointment or have not fully engaged in services.
* Motivate consumers to remain engaged and progress in treatment.
* Organize the timing and application of services to facilitate consumer success.
* Promote independence and develop external support structures to facilitate sustained
community integration.
2. Observe the development of strength based comprehensive intake screening, assessment;
treatment plans; modification of treatment plans
* Assist with comprehensive integrative assessment include and are not limited to: Physical health and dental health;
use of drugs or alcohol; education and employment; housing; transportation, resources;
social development and functioning; and family structure and relationships.
* Assist with ongoing assessment of clients' mental illness symptoms and clients' response
to treatment.
* Learn and practice documenting client progress to maintain a permanent record of client activity
according to established methods and procedures.
* Participate in weekly staff organizational meetings and treatment planning review
meetings.
3. Activities may involve learning how to assist clients with the following:
* Benefits (e.g., Supplemental Security Income [SSI], applying for Medicaid/Medicare
veterans' benefits, etc.)
* Activities of Daily Living including education on, personal hygiene and grooming tasks,
nutrition, meal planning, grocery shopping, and food preparation.
* Finding and Maintaining Safe and Affordable Housing - apartment hunting, finding a
roommate, landlord negotiations, cleaning, furnishing, and procuring necessities (e.g.,
telephone, furnishings, linens), and performing household activities, including house
cleaning and laundry.
* Grooming and Personal Hygiene, securing of appropriate clothing, wake-up calls,
transportation and teaching job-seeking skills.
* Money-Management Skills (e.g., budgeting and bill paying) and assist clients in accessing
financial services (e.g., professional financial counseling, emergency loan sources).
* Vocational Strengths and Problems, establish vocational or career goals and plans to
reach them, and recognize and target symptoms of mental illness that interfere with work.
* Substance Use concerns and the effects on mental and physical health and daily
functioning Participate in setting treatment goals and plans with clients to reduce or abstain
from substance use and to learn replacement behaviors
* Coordinate with outside inpatient services to detoxify clients and establish linkage to
outpatient treatment, self-help programs (e.g., Alcoholics Anonymous, Narcotics
Anonymous), outpatient services, and residential facilities when required
* Symptom education to enable clients to identify their mental illness symptoms.
* Accessing Reliable Transportation (e.g., obtain a driver's license and car, arrange for cabs,
access bus line, and find rides).
* Assist in developing appropriate social, interpersonal relationship and leisure time
activities.

EDUCATION & QUALIFICATIONS - CORE:
Enrolled in Bachelor's Degree in the social sciences field is required.
Must have a strong commitment to adhere to clients' right to have access to helpful, adequate, competent, and continuous supports
and services. Must possess a valid Oklahoma Driver License and satisfactory driving record and use
personal automobile to travel to locations other than primary office.
