Overview

To support the Mission of Helio Health, Inc., the Critical Time Intervention Director will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a "critical time" transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.

CTI Director:
Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. The Critical Time Intervention Director will oversee the new project implementation, be the point-person for engaging and providing education about the team and services provided to the community at-large. The Director will oversee the team of one clinician, two case managers, two peer case managers, and one nursing staff. The director will facilitate weekly team meetings, monitor implementation, and be responsible for data collection and performance outcome reporting. The Director will work on a Full-Time basis and using a hybrid work model, working in the community throughout Oneida County.

Schedule: Monday- Friday, 8:00am-4:30pm, with some evening on call responsibilities.

Pay Rate: $90,000.00 - $102,000 per year.

In addition to Helio Health's comprehensive benefits package, Program Directors are eligible for a Sign-On incentive up to $3,000-$8,000 depending upon licensure!



Responsibilities

    Maintains and provides supervision of program staff.
  • Implements Quality Improvement activities.
  • Fiscal program oversight and budget development.
  • Participates in Management and Administrative committees as assigned.
  • Assures program record documentation is in compliance with agency and regulatory standards.
  • Ensures that CTI workers' practice is consistent with the phase-specific activities and foci of the CTI GPD-CM model.
  • Works to create relationships with key providers in the community
  • Other duties as assigned by supervisor.


Qualifications

  • Qualified Health Professional (including but not limited to LMSW, LCSW, LMHC, etc.) required.
  • Minimum three years of clinical experience in an appropriate program setting.
  • Master's Degree in a related human service field required.
  • An equivalent combination of training and experience may be substituted for educational requirements.
  • Experience working with people experiencing homelessness

Salary and hourly compensation offers are based on candidate's education level and experience relevant to the position and also take into account information provided by the hiring manager and program.
